Job summary

Health Care Service Corporation is seeking a remote care coordination professional serving NM. The role involves telephonic and field care management to educate members on health programs, address health goals, and support clinical operations with providers.

Responsibilities include member health screenings, assessments, care planning, and possible home or facility visits to assist complex populations. Travel within NM region is expected; must have NM licensure and strong communication skills.

Qualifications

• RN or LPC/LMSW/LCPC/LCSW/LMFT with 2 years clinical practice.
• Current, valid, unrestricted NM license (or reciprocity).
• Plus 3 years wellness or managed care experience with clinical issues.
• Knowledge of health and wellness marketplace and employer trends.
• Verbal and written communication with members and staff.
• Analytical experience including medical data analysis.
• PC proficiency: Word, Excel, PowerPoint; database/Web apps.
• Valid driver's license and ability/willingness to travel.

Responsibilities

• Provide care coordination to members via telephonic and field-based care management.
• Inform and educate members on health care programs and goals.
• Complete health screenings, assessments, and care planning.
• Visit members' homes or facilities to support complex populations.
• Engage members in discussions about adherence and wellness.
