Qualifications

• Active RN or licensed mental health professional (LPC/LMSW/LCPC/LCSW/LMFT/LPCC/LISW/LSW/LMHC) license in the operating state.
• 3+ years wellness or managed care experience with clinical issues.
• Strong verbal and written communication with members and internal teams.
• Experience with data analysis and medical data utilization.

Responsibilities

• Provide care coordination to members via telephonic and field-based care management.
• Complete health screenings, assessments and care planning, potentially visiting homes or facilities.
