Zusammenfassung

HCSC is seeking a remote care coordination professional to support members via telephonic and field-based management. The role requires licensure (RN or related), with 3+ years of wellness or managed care experience and strong communication skills.

You will perform health screenings, assessments, and care planning, including potential home visits, and travel within the territory is required. This is a field-work role with hours 8 AM to 4:30 PM CST, Monday through Friday.

Qualifikationen

• This role requires RN, LPC, LMSW, LCPC, LMFT, LCSW, or related licensure with current, valid state license.
• Must have 3+ years of wellness or managed care experience addressing clinical issues with members.
• Strong verbal and written communication with internal/external stakeholders.
• Proficiency in Word, Excel, PowerPoint and related health IT tools.
