Job summary

Health Care Service Corporation is seeking an RN/LCSW/LPC-type professional to provide care coordination for members via telephonic and field-based care management. You will educate members on health care programs, discuss adherence to personal health goals, and address member inquiries while supporting clinical operations and provider activities.

Role requires complete health screenings, comprehensive assessments, and care planning, potentially including visits to members’ homes or facilities.

Qualifications

• Current, valid, unrestricted license in the state of operations.
• Plus 3 years’ wellness or managed care experience presenting clinical issues with members/physicians.
• Knowledge of the health and wellness marketplace and employer trends.
• Proficiency with Word, Excel and PowerPoint; database and web-based applications.
• Willingness to travel within assigned territory.
• Illinois-specific requirement: nursing/social work degree and 1 year supervised clinical experience for High-needs children.

Responsibilities

• Provide care coordination to members via telephonic and field-based visits.
• Educate members on health care programs addressing personal health needs.
• Engage members in discussing adherence to health goals and respond to inquiries from members.
• Support clinical operations, provider and member activities.
• Complete health screenings, comprehensive health assessments and care planning.
