Job summary

HCSC is seeking a care coordination professional to support members via telephonic and field-based care management. You will educate members on health programs, document progress, and engage them in personal health goals while supporting clinical operations and provider activities.

This remote role may involve home or facility visits and requires travel within the territory. Illinois-specific requirements apply for high-needs children.

Qualifications

• Current, valid RN license in state of operation or reciprocity.
• 3+ years of wellness or managed care experience with clinical issues.
• Knowledge of health and wellness marketplace and employer trends.
• Strong verbal and written communication with members and staff.
• Analytical experience with medical data.
• Computer proficiency: Word, Excel, PowerPoint; databases and web apps.

Responsibilities

• Provide telephonic and field-based care coordination to members.
• Educate members on health care programs and address personal health goals.
• Complete health screenings and comprehensive health assessments.
• Support clinical operations, provider and member activities.
• May require visits to members' homes or facilities.
