Qualifications

• Active, unrestricted license in the state of operations or reciprocity.
• 3 years’ wellness or managed care experience handling clinical issues with members/physicians.
• Strong verbal and written communication with internal and external stakeholders.
• Proficiency in Word, Excel, PowerPoint and database/web-based tools.

Responsibilities

• Provide telephonic and field-based care coordination for members.
• Educate members on health programs and address personal health goals.
• Complete health screenings, assessments, and care plans; travel as needed.
