Highmark Health is seeking an Advanced Practice Provider to deliver in-home, telehealth, and remote care in the Pittsburgh East area. This autonomous role focuses on geriatric and palliative populations, performing comprehensive clinical assessments and coordinating with the care team.

You will manage symptoms, support advance care planning, document risk adjustment, and ensure high-quality, patient-centered care across home and clinic settings.

Qualifications

• Graduation from an accredited PA program or an approved nursing master's/post-master’s program.
• Board certification in a relevant specialty is preferred (e.g., Family Medicine, Internal Medicine, Geriatrics, Hospice or Palliative Care).
• 3+ years of clinical experience as an NP or PA, preferably in primary care, geriatrics, palliative or hospice care, chronic disease management, home health, or similar community-based setting.

Responsabilités

• Conduct in-home, video, and telephonic comprehensive clinical assessments with detailed histories.
• Order and interpret labs and imaging; diagnose and plan therapy.
• Manage medications for palliative care and symptom relief.
• Guide patients and families on illness trajectory and goals of care.
• Facilitate advance care planning and end-of-life discussions.
• Coordinate care with the patient’s care team and participate in interdisciplinary meetings.
