Job summary

Alignment Health is seeking an Advanced Practice Provider (NP/PA) to manage complex patients in a home-based care setting. You will collaborate with physicians, social workers, and care managers while conducting in-home assessments and some outpatient work.

Qualifications

• Minimum experience: at least 1 year clinical experience.
• Active state NP/PA license with license-specific requirements at hire.
• Furnishing number and NPI/DEA numbers required within designated timeframes.

Responsibilities

• Conduct in-home assessments (3.5 days/week) with some outpatient clinic work.
• Perform comprehensive history, physical exam, med review, and cognitive/safety screenings.
• Discuss end-of-life care with patients and families as needed.
• Collect laboratory specimens in home setting when appropriate.
• Coordinate with PCP and care team to identify care gaps and prioritize follow-up.
