Alignment Health is seeking a Field Remote Advanced Practice Clinician to perform in-home assessments and virtual visits for seniors with complex care needs. You will collaborate with physicians, case managers and other clinicians to design and implement effective, cost-conscious care plans.

The role emphasizes patient education, care coordination and leadership of a clinical team in a fast-paced environment while maintaining regulatory compliance and high-quality outcomes.

Qualifications

• Required: Master’s degree from an accredited NP or PA program.
• Active Nurse Practitioner or Physician Assistant license for NC.
• Excellent communication, teamwork and time management.

Responsabilités

• Perform in-home assessments for Alignment members with 45–60 minute visits.
• Provide patient education aligning care needs with best options.
• Identify diagnoses for care management and medical management.
• Coordinate care with PCPs, specialists and ancillary providers.
• Lead a clinical team including nurses, health coaches and care coordinators.
