Job summary

HarmonyCares is seeking an Advance Practice Provider (NP/PA) to manage chronic conditions for high-risk geriatric patients in home settings. The role emphasizes patient-centered care with a focus on quality outcomes within a physician-driven, team-based model.

Ideal candidates hold NP or PA licensure in the state, with DEA eligibility, CPR/BLS certification, and geriatric experience. Competitive benefits accompany a base salary and CME support.

Qualifications

Licensed NP or PA in the state where HarmonyCares Medical Group operates.
Federal DEA license/eligibility.
Geriatric training/experience preferred.
Active CPR/BLS Certification.
Board Certification.
Must maintain a valid driver's license and clean driving record.

Responsibilities

Provide home-based primary care to high-acuity geriatric patients Monday–Friday.
Practice medicine focused on patient care over volume.
Take time to address patient needs and coordinate with the care team.
Collaborate with nurses, specialists, and care coordinators to optimize outcomes.
