Qualifications

• Graduate from an accredited PA program OR graduate from a Nursing approved Master’s or Post Master’s program.
• Board certification or clinical experience in a relevant specialty preferred.

Responsibilities

• Perform comprehensive in-home, video, and telephonic assessments with detailed histories, exams, and documentation of active diagnoses and social determinants.
• Order/interpret labs and imaging; diagnose and initiate appropriate therapy within scope and standards of practice.
• Manage palliative care medications and symptoms; provide education and support to patients and families.
