Pine Park Health Management Inc. is seeking a Certified Medical Assistant to serve as the Remote Primary Care Coordinator.

You will be the operational and relational backbone of our geriatric primary care team, coordinating care for older adults across AZ, CA, and NV from home. In this fully remote role, you will triage urgent needs, maintain accurate EMR documentation, schedule follow-ups, onboard new patients within 24 hours, and collaborate with families, facility staff, and providers to

Qualifications

• 2+ years of direct patient care experience in a clinical setting.
• Bachelor’s degree strongly preferred.
• Experience in healthcare coordination or clinical operations.
• Excellent triage and prioritization in real-time environments.
• Proficiency with EMR/EHR systems.
• Strong communication across patients, families, physicians and staff.
• Able to work independently in a fully remote setting.

Responsibilities

• Serve as primary point of contact for patients, families, facility partners, and providers.
• Triage urgent care requests and time-sensitive clinical communications.
• Maintain accurate patient documentation within the EMR system.
• Coordinate scheduling and follow-up care across care teams.
• Lead onboarding and enroll new patients within 24 hours.
• Support after-hours on-call rotation for continuity of care.
• Contribute to quality improvement and efficiency initiatives.
