Job summary

Pine Park Health Management Inc. is seeking a Remote Clinical RN licensed in California to deliver geriatric primary care from home. This fully remote role supports residents of senior living communities across California, Nevada, and Arizona, collaborating with PCPs, Medical Directors, and Medical Assistants.

As the hub of the care team, you will triage communications, coordinate care plans, and participate in crisis and palliative discussions while advancing quality improvement initiatives.

Qualifications

• Current unrestricted multi-state RN license with at least 2 years of clinical experience, preferably in geriatrics or related fields.
• Experience in remote roles, showcasing proficiency with technology and digital communication platforms.
• High accountability, exceptional communication, and ability to thrive in fast-paced environment.
• Experience with geriatric patients, particularly in senior living communities or skilled nursing facilities.
• Familiarity with chronic care management and geriatric diseases and treatments.
• Collaborative mindset, valuing contributions of a multidisciplinary team and committed to improving patient outcomes.

Responsibilities

• Collaborate with PCPs to support a large patient panel.
• Triage inbound communications via calls, faxes, email and messages; close out, escalate to PCPs when needed, and route sub-tasks to Medical Assistants.
• Support LVN/LPNs within the team to improve efficiency and effectiveness.
• Prepare charts and educate patients on care plan adjustments.
• Oversee post-appointment processes, including diagnostic orders, referrals, and medication orders.
• Participate in daily huddles and coordinate with PCPs and MA teammates.
• Conduct virtual patient assessments and develop personalized care plans.
• Serve as a key communication link among patients, PCPs, specialists and other healthcare professionals.
• Promote preventive care and self-management strategies aligned with care plans.
• Participate in crisis intervention and palliative care discussions.
• Contribute to ongoing quality improvement initiatives and geriatric care research.
