• Bachelor's Degree or at least 4 years of professional experience in disabilities services.
• Strong organizational, interpersonal, communication, and leadership skills.
• Proficient with Microsoft Office and related software.

Responsabilités

• Perform initial and ongoing assessments using approved tools and procedures.
• Evaluate functional status, service needs, supports, and risks.
• Complete all required documentation accurately and on time.
• Communicate assessment findings and follow-up needs to designated parties.
• Coordinate with members, caregivers, case managers, MCOs, and agency staff.
• Maintain compliance with policies, confidentiality rules, and reporting expectations.
• Participate in training, quality assurance, and calibration activities.
