Qualifications

• Active license required: RN, LISW, LMHC, or Licensed Psychologist.
• Experience with assessments and care coordination preferred.
• Strong communication and organizational skills.
• Must have reliable transportation for regional travel.

Responsibilities

• Conduct comprehensive clinical assessments and reassessments.
• Develop personalized care plans and connect clients to services.
• Document findings accurately in required systems and maintain records.
• Coordinate with members, caregivers, providers, and case managers.
• Adhere to CareStar policies and confidentiality requirements.
• Travel within the region as needed.
