Job summary

CareStar, Inc. is seeking a Generalist Assessor to support the Iowa Clinical Department. The role blends remote work with field visits, assessing needs and building individualized care plans for individuals in Black Hawk County and surrounding areas.

You will document findings, coordinate with care teams, and ensure compliance with policies while delivering compassionate, person-centered services to improve lives.

Qualifications

• Bachelor's Degree or 4+ years in IDD/physical disabilities.
• Strong organizational, interpersonal, communication and leadership abilities.
• Proficient with Microsoft Office suite.
• Reliable transportation and valid driver's license.

Responsibilities

• Perform initial and ongoing assessments using approved tools.
• Evaluate functional status, service needs, supports, and risks.
• Complete documentation accurately and on time.
• Communicate assessment findings to designated parties and coordinate with care teams.
• Maintain compliance with policies and participate in training.
