Job summary

CareStar, Inc. is seeking a Generalist Assessor to join the Iowa Clinical Department. This full-time role combines remote work with field visits to assess needs, create care plans, and connect individuals with essential services.

You will perform assessments, coordinate with care teams, and ensure documentation and confidentiality compliance, supporting healthier, more independent lives in surrounding counties.

Qualifications

• Bachelor's Degree or 4 years of professional experience in intellectual, developmental, and/or physical disabilities.
• Strong organizational and interpersonal skills; ability to communicate clearly.
• Proficiency with Microsoft Office.

Responsibilities

• Performs initial and ongoing assessments using approved tools and procedures.
• Evaluates functional status, service needs, supports, and risks.
• Completes all required documentation accurately and on time.
• Communicates assessment findings and follow-up needs to designated parties.
• Coordinates with members, caregivers, case managers, MCOs, and agency staff as needed.
• Maintains confidentiality and complies with policies.
