Job summary

California Center for Functional Medicine (CCFM) seeks an experienced Health Coach for 10-20 hours per week, remote, temporary maternity coverage with potential for extension. You will translate practitioner recommendations into personalized lifestyle plans and provide accountability in a fully virtual, patient-centered setting.

Ideal candidates have formal health coach training, experience with functional medicine, and are comfortable using telehealth tools and electronic records to support

Qualifications

• Professional training or certification in health coaching.
• Experience providing individualized health coaching, wellness coaching, or lifestyle behavior-change support.
• Functional medicine experience preferred, especially with chronic conditions.
• Nutrition education or experience is a plus but not required.
• Strong communication and ability to work independently in a remote setting.
• Ability to learn new systems quickly and adapt to workflows.

Responsibilities

• Conduct virtual health coaching consultations to assess goals, habits, challenges, and progress.
• Translate practitioner recommendations into personalized treatment, nutrition, and lifestyle plans.
• Provide ongoing motivation, accountability, and support through regular follow-ups.
• Educate patients on wellness principles, healthy habits, stress management, sleep, and behavior change.
• Monitor progress, identify barriers, and collaborate with practitioners to adjust plans.
• Act as patient navigator to keep patients engaged and guide next steps.
• Maintain accurate, confidential patient records and adapt to practice workflows.
• Collaborate with clinical team and contribute to patient education materials as scheduling permits.
