Evolving Solution Services in Carmichael, CA is seeking a Full-Time Professional APC to provide comprehensive medical evaluation and ongoing management of SNF residents within California license scope. • Nurse Practitioner (NP): Must maintain a current and active Nurse Practitioner license and all certifications required for the position.
• Physician Assistant (PA): Must maintain a current and active Physician Assistant license and all certifications required for the position.

Responsibilities

• Provide comprehensive medical evaluation and ongoing management of SNF residents within the APC’s California legal scope, clinical competency, and facility privileges.
• Perform admission evaluations, history and physical examinations, and post-hospitalization assessments as permitted by applicable law and facility requirements.
• Diagnose and manage acute and chronic medical conditions, including changes in resident conditions.
• Develop, implement, and update individualized treatment plans.
• Order, review, and interpret appropriate laboratory tests, diagnostic studies, and other medically necessary evaluations.
• Prescribe, furnish, and/or order medications and treatments as permitted by California and federal law and the APC’s applicable practice authority.
• Perform medication reconciliation and monitor medication effectiveness, adverse effects, and potential interactions.
• Evaluate and manage acute changes in condition, falls, infections, respiratory issues, abnormal laboratory results, pain, dehydration, and other medical problems.
• Determine when escalation of care, emergency evaluation, specialist consultation, or hospital transfer is clinically indicated.
• Provide appropriate follow-up after hospital or emergency department visits.
• Participate in interdisciplinary care planning, including coordination with nursing, rehabilitation, dietary, pharmacy, social services, specialists, residents, and families.
• Communicate with residents and families regarding diagnoses, treatment plans, clinical status, and goals of care.
• Participate in advance-care planning, POLST discussions, hospice/palliative-care coordination, and end-of-life care as appropriate.
• Maintain timely, accurate, and complete medical documentation, including progress notes, assessments, orders, and treatment plans.
• Collaborate with the attending/supervising physician and other healthcare professionals and communicate significant clinical changes in accordance with facility policy and applicable law.
• Participate in quality improvement, infection prevention, medication management, and other facility initiatives relevant to resident care.
• Provide appropriate coverage and clinical communication during assigned on-call or after-hours responsibilities, consistent with facility policy.

Full-Time Professional Carmichael, CA, US

Salary Range: $75.00 To $90.00 Hourly

Position: AIMS SNF (Skilled Nursing Facility) Services APC

Location: Assigned SNFs- subject to change per availability

Expected Work Schedule

• Physical Visits: Monday–Friday, 8:00 AM–5:00 PM as assigned
• Day Calls: Monday–Friday, 7:00 AM–7:00 PM as assigned
• On-Call:
• Minimum of 3 shifts per month, as needed.
• Monday–Friday: 7:00 PM–7:00 AM
• Saturday–Sunday: 7:00 AM–7:00 AM

Position-Specific Duties and Responsibilities

• APCs will touch base with the Unit Manager during every facility visit to discuss changes in condition, new orders, plans of care, and other patient-related concerns.
• APCs will sign all prescriptions daily. They should contact the Unit Supervisor regarding anyorders received from the pharmacy that require a signature.
• APCs will check the binder at every visit and address necessary labs, questions, signatures, and other items requiring attention in the binders located ateach nurse’s station.
• APCs will electronically prescribe/fill prescriptions whenever initiating a patient on a new CII medication.
• If an APC is unable to report to the facility on their assigned day for any reason, they must notify the SNF Coordinator, Supervising Physician, and Unit Supervisor at the facility.
• APCs may independently prescribe antidepressant medications but must consult with their Supervising Physician before initiating anyantipsychotic medication.
• The APC’s scope of practice is not limited to the duties listed above and includes medical management consistent with the APC’s education, training, qualifications, and applicable scope of practice.
• Charts must be completed within 24 hours in AIMS’s EMR, then copied and pasted into the facility’s EMR. The billing log must be submitted to the billing department via email daily .

Scope & Accountability

APCs shall perform services only within their California license, certification, applicable practice agreement or statutory authority, clinical competency, and facility-granted privileges . Nothing in this job description supersedes California law, federal regulations, Title 22 requirements, facility policies, or the attending/supervising physician’s responsibilities.

For PAs: Services are provided pursuant to the applicable California PA–physician Practice Agreement and supervision requirements.

For NPs: Services are provided pursuant to the NP’s applicable California practice authority, including standardized procedures or independent practice authority when applicable.
