Job summary

US Health Partners, LLC is seeking a Nurse liaison to coordinate care and communicate medical information among the Injured Worker, the Insured, and the Carrier to facilitate appropriate treatment and return to work. This remote, field-based role involves regular travel within the region.

Qualifications include an active RN license, possible CCM/CRC/CLNC/CRRN certification within 3 years, and at least two years of direct case management for injured workers.

Qualifications

• RN license to practice in applicable states.
• Must obtain and maintain at least one national certification (e.g., CCM, CRC, CLNC, CRRN) within 3 years of hire or state timeframe.

Responsibilities

• Follow CM plan policies and ensure CM process accountability.
• Accept referrals from Director of Case Management.
• Schedule initial assessment and obtain next physician appointment date.
• Maintain communication among Injured Worker, Insured, and Carrier.
• Record data in CaseAnyplace and submit monthly reports.
• Attend physician appointments and document diagnosis, prognosis, and treatment plan.
• Coordinate transportation and refer to Vocational Counselor when appropriate.
• Assist with RTW planning and provide information to Defense Attorneys as needed.
• Promote teamwork and ensure accurate billable hours.
