Job summary

US Health Partners, LLC is seeking a Nurse liaison to coordinate care and share pertinent medical information among the Injured Worker, the Insured, and the Carrier. This remote, field-based role requires local travel within the region and supports rehabilitation, cost-efficient care, and timely return to work.

You will engage in case management activities across work injuries, motor vehicle accidents, liability claims, and other referred services, ensuring compliant documentation and ongoing

Qualifications

• Current, unrestricted RN license; licensed in each state where field case management is provided.
• Must obtain and maintain a nationally recognized certification (e.g., CCM, CRC, CLNC, CRRN) within three years or state timeframe.
• Two years full-time direct case management experience for injured workers preferred.
• Completion of a nursing program with ongoing CE as required.

Responsibilities

• Coordinate care and communicate among Injured Worker, Insured, and Carrier to facilitate treatment and return to work.
• Record data and billing in CaseAnyplace; submit monthly reports (Preliminary, Initial, Progress, Closing).
• Attend physician appointments to gather diagnosis, prognosis, treatment plan, rehab length, and RTW details.
• Promote teamwork with all staff and maintain licensure and annual training.
