National Care Advisors is seeking a Nurse Case Manager based in Austin to coordinate case management for eldercare clients, addressing quality-of-life concerns and collaborating with families, attorneys, and trustees. The role supports a flexible, work-from-home schedule with regional travel as needed.

Requirements include a BSN, 5+ years of nursing experience, and CCM or CRRN preferred. Candidates should be proficient in MacOS and MS Office, with strong communication and organizational skills.

Qualifications

• BSN is required.
• 5+ years clinical nursing experience is required; field case management or workers’ compensation experience preferred.
• Experience with third party benefits (health insurance, Medicaid, Social Security, DD services).
• Excellent written and verbal communication skills.
• Proficient in using MacOS and MS Office applications.
• Ability to learn new technologies in a Mac environment.
• Self-motivated with strong time management and organization.
• Collaborative approach to achieve goals and resolve conflicts.
• Research and develop solutions for client challenges.
• Excellent customer service skills.
• Willingness to travel regionally 1-2 times per month and occasionally nationally.
• Commitment to timely documentation.

Responsibilities

• Provide case management services for eldercare clients and special needs.
• Collaborate with primary family caregivers, attorneys, financial planners, and trustees.
• Maintain timely documentation and reporting.
• Demonstrate flexibility with a work-from-home schedule.
• Travel regionally 1–2 times per month and occasionally nationwide.
