Job summary

National Care Advisors is seeking a Nurse Case Manager in the DC/Baltimore area to coordinate case management for eldercare clients, working with families, attorneys, and trustees to support quality of life. The role emphasizes timely documentation, strong communication, and cross-functional collaboration to meet client needs.

Requirements include a BSN, 5 years of nursing and case management experience, and proficiency with MacOS and MS Office.

Qualifications

• Bachelor of Science in Nursing (BSN) required, CCM or CRRN preferred.
• Minimum of 5 years clinical nursing experience with case management preferred.
• Experience with third party benefits including health insurance, Medicaid, and Social Security.

Responsibilities

• Manage case coordination for eldercare clients including collaboration with families, attorneys, financial planners and trustees.
• Ensure timely documentation and regulatory compliance.
• Communicate effectively in writing and verbally with clients and care teams.
• Travel regionally 1–2 times per month and nationally on occasion.
