Job summary

NCA is seeking a Nurse Case Manager based in Austin to manage casework for special needs and eldercare clients, coordinating with families, attorneys, financial planners and trustees to support quality of life.

Requirements include a BSN and 5+ years of clinical nursing experience in case management; CCM or CRRN preferred. The role offers a salaried position with a flexible work-from-home schedule and regional/national travel as needed.

Qualifications

• BSN required; CCM or CRRN preferred.
• Minimum 5 years clinical nursing experience in case management.
• Experience with third party benefits (health insurance, Medicaid, Social Security).
• Excellent written and verbal communication skills.
• Proficient with MacOS and MS Office; quick learner with new tech.

Responsibilities

• Provide case management services for special needs and eldercare clients.
• Collaborate with primary family caregivers, attorneys, financial planners, and trustees.
• Maintain timely documentation and meet regulatory requirements.
• Flexible, work-from-home schedule with regional travel 1–2 times per month and some nationwide travel.
