Job summary

NCA seeks a Nurse Case Manager based in San Diego to coordinate case management for eldercare clients and complex care needs. You will collaborate with family caregivers, attorneys, financial planners and trustees to support client outcomes.

Requirements include a BSN and 5+ years nursing experience; CCM/CRRN preferred. Proficiency in MacOS and MS Office, strong communication, and willingness to travel regionally 1–2 times monthly are essential.

Qualifications

• BSN required; CCM or CRRN preferred.
• Minimum 5 years clinical nursing experience; case management experience preferred.
• Experience with third-party benefits (health insurance, Medicaid, Social Security, Developmental Disability services).
• Excellent written and verbal communication skills.
• Proficient in MacOS and MS Office.
• Able to learn new technologies in a Mac environment.
• Self-motivated and able to work in a virtual company; strong time management.

• Overnight travel regionally 1–2 times per month and occasional nationwide travel.
• Willingness to meet timely documentation requirements.

Job description

NCA is in search of a Nurse Case Manager based in San Diego who will be responsible for case management services responsive to special needs and eldercare client care and quality of life challenges and collaboration with the primary family caregivers, attorneys, financial planners and trustees that are also serving the client.

Requirements

• Bachelor of Science in Nursing (BSN) required, CCM or CRRN preferred
• Minimum of 5 years clinical Nursing experience - experience in field case management or workers’ compensation case management, preferred
• Experience with third party benefits - health insurance, Medicaid, Social Security, Developmental Disability services
• Excellent communication skills - written and verbal
• Proficient in using MacOS and a variety of related software applications (including MS Office)
• Strong ability to quickly learn and adapt to new technologies and tools in a Mac environment
• Highly motivated self-starter comfortable working in a virtual company/office, with solid time management and organization skills
• Proven ability to collaborate as necessary to accomplish goals and work through conflicts
• Ability to research and develop solutions to challenges presented by the client
