Job summary

National Care Advisors is seeking a Nurse Case Manager based in Delaware to lead case management for eldercare clients, coordinating care with families, attorneys, financial planners, and trustees to improve quality of life.

Requires a BSN and 5+ years of clinical nursing experience, with CCM/CRRN preferred. Strong communication, MacOS/MS Office proficiency, and ability to work remotely with regional and occasional nationwide travel are essential.

Qualifications

• BSN is required; CCM or CRRN preferred.
• Minimum of 5 years clinical nursing experience; case management or workers’ compensation case management preferred.
• Experience with third party benefits – health insurance, Medicaid, Social Security, Developmental Disability services.
• Excellent written and verbal communication skills.
• Proficient in using MacOS and MS Office; able to learn new technologies in a Mac environment.
• Strong time management and organizational skills in a virtual work setting.
• Ability to collaborate to achieve goals and navigate conflicts.
• Ability to research and develop solutions for client challenges.
• Excellent customer service skills.
• Travel: overnight regional 1–2 times per month and occasional nationwide travel.
• Willingness to meet timely documentation requirements.

Responsibilities

• Provide nurse case management services responsive to special needs and eldercare client care and quality of life challenges.
• Collaborate with primary family caregivers, attorneys, financial planners and trustees serving the client.
• Manage documentation and ensure timely reporting to stakeholders.
• Support clients in accessing third-party benefits and community resources.
