Job summary

Sedgwick is seeking a Field Case Manager in Windsor, CT. You will work face to face with injured workers and medical providers to advocate for timely, effective treatment and a smooth return-to-work process, with travel required but a home-based office supported.

Your background in health or rehabilitation, nursing preferred, and certifications like CCM, CDMS, or CRC help you coordinate care, educate families, and liaison among all parties to optimize outcomes.

Qualifications

• RN licensure preferred or bachelor's degree in health/human services.
• CCM, CDMS, or CRC certifications are valued.

Responsibilities

• Serve as patient advocate to support, guide and coordinate care for injured workers and families.
• Assist injured workers in recovery through advocacy, communication, education, and resource facilitation.
• Identify providers and facilities to ensure timely, cost-effective treatment across the continuum.
