Job summary

UnitedHealthcare seeks a Field Care Coordinator in Idaho to remotely manage a panel of members with complex health needs, blending home-based work with up to 50% field visits to coordinate care and improve outcomes from a patient-centered perspective.

You'll partner with care teams, providers, and community resources to implement tailored care plans, support HCBS, and guide members through transitions while maintaining PHI security and compliant practice.

Qualifications

• Idaho license in a clinical field (RN or a qualifying license).
• 1+ year Medicaid/Medicare or SDoH experience.
• 1+ year MS Office (Word/Excel/Outlook).
• Quiet home workspace with PHI protection.
• Idaho residence with reliable internet.
• Eligible for travel within territory up to 50%.

Responsibilities

• Serve as primary care manager for dual eligible members.
• Conduct comprehensive needs or wellness assessments (in person or telephonic).
• Collaborate with care team, providers, and community resources to implement plans.
• Assist members with HCBS support and services.
• Provide referrals to internal and community resources as appropriate.
• Support discharge planning and care transitions after ER/Facility stays.
• Educate and coach on self-management and lifestyle changes.
• Advocate for member needs within the health care team.
