Responsibilities

• Visit Medicaid members in homes, facilities or LTC settings (75-90% travel).
• Assess and evaluate member needs to create a care plan.
• Ensure services are in least restrictive setting for wellbeing.
• Plan and implement interventions to meet needs.
• Coordinate services and monitor the care plan against goals.
• Guide members and families to appropriate resources.
