Job summary

Humana is seeking a Field Care Coordinator to support Medicaid members in Orange County, FL. You will assess needs, create care plans, and connect members with suitable services to promote wellness and positive health outcomes.

Responsibilities include home visits (75-90% travel), coordinating with providers, and ensuring services are delivered in the least restrictive setting. Requires Florida RN/LPN or equivalent, 2+ years case management, and bilingual English/Spanish.

Qualifications

• Bachelor's degree with ≥2 years in Health Services, Social Work or Psychology.
• RN or LPN license in Florida with relevant experience.
• Medicare & Medicaid experience preferred.
• Minimum 2 years of case management experience.
• Bilingual English/Spanish preferred.
• Schedule: Monday-Friday 8:30am-5:00pm.
• AHCA fingerprint clearance required.
• TB screening participation is required.
• Valid state driver's license and personal vehicle insurance.

Responsibilities

• Visit Medicaid members in homes, ALFs, and LTCFs with substantial local travel.
• Assess and establish member-specific care plans.
• Ensure services in least restrictive settings to optimize wellbeing.
• Plan and implement interventions to meet needs.
• Coordinate services and monitor the care plan against goals.
• Guide members and families to appropriate resources.
• Interact with payers, providers and interdisciplinary teams as needed.
