Job summary

Humana is seeking a Care Coach 1 in Florida to visit Medicaid members in homes and facilities, assess needs, and create personalized care plans. You will connect members with essential services to promote wellbeing and health outcomes.

The role emphasizes teamwork, education on health and wellness, and engagement with diverse communities. Requires valid FL driver’s license and TB screening compliance.

Qualifications

• Bachelor's Degree with at least 2 years of relevant experience in health services, social work or psychology.
• RN with at least 2 years of relevant experience in Florida.
• LPN in Florida with at least 4 years of relevant experience.
• Case Worker with 6+ years of health services/case management experience.
• Experience with Medicare & Medicaid recipients.
• Proficient MS Word, Excel, Outlook.
• AHCA fingerprint required.
• TB screening as part of Humana process.
• Valid state driver’s license and vehicle insurance.

Responsibilities

• Visit Medicaid members in homes, facilities or care settings with 75-90% local travel.
• Assess and establish member-specific care plans.
• Ensure services are received in least restrictive settings.
• Plan and implement interventions to meet needs.
• Coordinate services and monitor progress toward goals.
• Guide members and families to appropriate resources.
• Facilitate interactions with payers, providers, and care teams.
