Responsibilities

• Ensure services are provided in the least restrictive setting.
• Plan and implement interventions to meet member needs.
• Coordinate services and monitor the care plan against goals.
• Guide members and families to appropriate resources.
• Collaborate with payers, providers, and interdisciplinary teams.
