Qualifikationen

• Graduate of an accredited school of nursing.
• 3+ years in case management or similar role with individuals with chronic illnesses.
• 2+ years in community health setting.
• 2+ years in clinical, hospital, acute care, or direct care setting.
• 2+ years using MS Word, Excel, and Outlook.

Aufgaben

• Conduct telephonic or face-to-face holistic evaluations of members' needs and preferences.
• Provide education and support to members on service delivery models.
• Perform initial and follow-up assessments and outreach calls.
• Identify members for high-risk complications and coordinate care.
• Manage members with chronic illnesses and disabilities for efficient health benefit utilization.
• Assess, plan, and implement individualized care strategies.
• Utilize company and community resources for effective case management.
• Collaborate with members, families, and healthcare providers to develop care plans.
• Identify and initiate referrals for social service programs.
• Manage care plans throughout the continuum of care.
• Communicate health-related information to ensure quality care.
• Advocate for patients and families to represent their needs.
• Utilize clinical criteria to assess appropriate levels of care.
• Document member assessments, care plans, and referrals.
• Participate in interdisciplinary team meetings and utilization management rounds.
• Monitor services for timeliness and appropriateness.
