Job summary

Humana is seeking an RN, Field Care Manager 2 (Physical Health) to assess member needs, develop care plans, and coordinate services across the care team. This role operates primarily in the field with remote/home office duties when not visiting members, requiring up to 50% travel within Louisiana regions.

Strong EMR skills and independent judgment are essential. Preferred BSN and CCM certification, with experience in health promotion and bilingual capabilities being a plus.

Qualifications

• Active, unrestricted RN license in Louisiana.
• 2+ years clinical experience with diverse populations.
• Knowledge of community health and social service agencies.
• Experience with electronic medical records and narrative documentation.
• Proficient in Word/Outlook/Excel.
• Ability to work independently and adapt care plans as needed.

Responsibilities

• Engage and empower members and families to support recovery and wellness.
• Conduct telephonic and in-person assessments; guide to resources.
• Monitor progress toward individualized care goals.
• Develop and implement personalized care plans.
• Collaborate with healthcare providers and community services.
• Coordinate delivery of essential services across health determinants.
• Facilitate communication across the care team for seamless transitions.
• Align daily practice with organizational objectives.
• Document and submit incident reports per policies.
