Job summary

UnitedHealth Group through Optum is seeking a Field-Based Community Health Worker to engage health plan members in their homes and communities. This full-time role focuses on improving access to care, education on benefits, and coordinating visits with providers.

You will work from a home-based office in Oklahoma City and surrounding areas, with a 40-hour work week, Monday through Friday, Central Time, and occasional after-hours support as needed.

Qualifications

• 3+ years of experience working within a community health setting or community outreach experience.
• 2+ years of professional experience working with diverse multi-ethnic and socioeconomic populations.
• 1+ years of field-based experience.
• Intermediate level of computer experience with Windows and Excel.

Responsibilities

• Engage members at homes or community settings to promote program engagement.
• Educate members about program benefits and plan offerings.
• Coordinate and schedule clinical visits to ensure timely care access.
• Build trust with members and connect them to Optum providers.
• Plan routes to optimize field visit schedules and travel time.
