Qualifications

• Graduate from an Accredited School of Nursing if holding clinical licensure.

Responsibilities

• Evaluates member needs, barriers to care, and recommends care plans.
• Develops or contributes to personalized care/service plans and coordinates providers.
• Provides psychosocial support to members and caregivers and connects to local resources.
• Coordinates between member/family and care provider team to ensure timely access to services.
• Monitors progress toward goals and collaborates with providers for plan revisions.
• Documents and maintains all member information to ensure regulatory compliance.
• May perform on-site visits and educate care managers and families on procedures and benefits.
• Adheres to all policies and standards.
