Job summary

Centene is seeking a field-based Care Manager to assess and coordinate care for members in Oklahoma. You will develop personalized care plans, connect members with community resources, and educate families and caregivers to improve access to high-quality care.

Qualifications

  • Bachelor's degree and 2–4 years of related experience.
  • Current state's clinical nursing license preferred.
  • Strong communication, organization, and time management abilities.

Responsibilities

  • Evaluates the member's needs, barriers to care, resources, and coordinates the plan for the best outcome.
  • Develops or contributes to personalized care/service plans and identifies providers or community resources needed for care.
  • Provides psychosocial and resource support to members, caregivers, and care managers to access local resources or services.
  • Coordinates between the member/family and the care team to ensure services are accessible in a timely manner.
  • Monitors progress toward care plan goals and collaborates with providers for revisions as needed.
  • Collects, documents, and maintains all member information to ensure compliance with regulations.
  • May perform on-site visits to assess needs and collaborate with providers or resources.
  • Educates care managers and/or members on procedures, instructions, care options, referrals, and benefits.
