Qualifications

• Master’s degree in a health-related field or human services.
• Active professional license as applicable (RN/LCSW/LCMHC/LMFT/LMFTA, etc.).
• Ability to drive and travel within NC as needed.

Responsibilities

• Conduct Health Risk Assessments and synthesize a holistic care plan.
• Coordinate across multiple health domains with an interdisciplinary team.
• Maintain and update Care Plans and documentation in the AHR system.
• Provide transitional and crisis management support to members and caregivers.
• Engage members and families to identify barriers and ensure service access.
