Job summary

Community Care of North Carolina Inc is hiring a Care Manager 1 - Non-Clinical for statewide coverage across 100 NC counties. The role requires residency in NC or within 40 miles of the NC border, with field-based duties and periodic remote work when not in the field.

The position focuses on coordinating integrated health care for Medicaid enrolled members, collaborating with primary care providers, families, and community resources. Travel within NC is required.

Qualifications

  • Bachelor's degree in health, psychology, sociology, social work, nursing or related field (or RN licensure).
  • 2 years of experience working with individuals served by child welfare preferred.
  • Must reside in NC or within 40 miles of the NC Border.
  • CCM certification preferred.
  • Maintain a valid driver's license and auto liability insurance.

Responsibilities

  • Provide statewide non-clinical care management across multiple counties.
  • Assess and plan to ensure integrated, coordinated health care for Medicaid enrolled members.
  • Coordinate with PCPs, caregivers, and care team to develop a comprehensive care plan.
  • Educate members and families about diagnoses, medications, and available resources.
  • Monitor care plan effectiveness and progress toward SMART goals.
  • Utilize EMR to document care activities and referrals.
  • Travel within NC as required; remote work when not in the field.
