Qualifications

• Experience with NC Medicaid or Tailored Plans preferred.
• Strong clinical assessment and care planning skills.
• Ability to work remotely within NC with travel as needed.

Responsibilities

• Conduct comprehensive Health Risk Assessments and develop person-centered Care Plans.
• Coordinate services across medical, behavioral health, LTSS and supports networks.
• Collaborate with an interdisciplinary team to ensure quality care and compliance.
• Support transitions and crisis planning as part of care management.
• Monitor and document care activities in the AHR system with accuracy.
