Job summary

Community Care of North Carolina, Inc. is seeking a Care Manager 1 - Non-Clinical in Forsyth County. The role supports Medicaid members statewide, with remote work when not in the field and occasional travel for training.

Must reside in NC or within 40 miles of the NC border. A Bachelor’s degree in a health-related field is required, with CCM certification preferred. This is a full-time position based in Winston-Salem, NC, offering competitive benefits.

Qualifications

• Requires a Bachelor's Degree in a health/human services field or licensure as an RN.
• 2 years of experience with child welfare population preferred.
• Must reside in NC or within forty miles of the NC border.
• CCM certification preferred.
• Maintain a valid driver’s license with current auto liability insurance.

Responsibilities

• Provide holistic, integrated care management across physical health, behavioral health, LTSS, and other services.
• Complete member assessments addressing medical, biopsychosocial, and cultural needs.
• Collaborate with members, guardians, and families to address barriers and support self-management.
• Educate members and families about diagnoses, medications, resources, and preventative care.
• Monitor progress and outcomes, setting SMART goals with members and families.
• Develop and evaluate member care plans with the care team and providers.
• Coordinate referrals to community-based services and ensure care continuity.
