Job summary

Cognizant is seeking an ER Medical Coder for E/M, injections & infusions. This remote role supports healthcare clients, focusing on accurate ICD-10-CM coding for ED, outpatient surgery, observation, and diagnostic encounters.

You will collaborate with providers and coding teams, ensure regulatory compliance, and maintain a 98% accuracy rate while meeting productivity targets in a fast-paced remote environment.

Qualifications

• Certifications: CCS, RHIT, RHIA (AHIMA), CPC, or COC (AAPC).
• Minimum 2 years facility-based outpatient coding experience.
• Experience coding Emergency Department encounters (facility E/M).
• Experience with injections and infusion coding in outpatient or hospital settings.
• Knowledge of federal, state, and payer regulations related to medical coding and reimbursement.

Responsibilities

• Review clinical documentation and assign ICD-10-CM diagnosis and procedure codes for outpatient settings.
• Use encoder applications to assign codes per policy and regulations.
• Query physicians to clarify diagnoses/procedures when needed.
• Perform peer reviews to support quality and consistency.
• Prepare and complete coding-related reports as requested.
• Maintain coding accuracy rate of 98% while meeting productivity standards.
• Ensure compliance with coding guidelines and regulations.
• Work remotely while managing multiple systems and priorities.
