Job summary

Cognizant is hiring an ER Medical Coder – E/M, Injections & Infusions to support healthcare clients remotely. You will review clinical docs, assign ICD-10-CM codes for ED, outpatient surgery, observation, and diagnostic encounters, while ensuring compliance with regulations.

The role requires 2+ years of facility outpatient coding, ED/E/M experience, and proficiency with encoder tools. Strong communication with physicians is essential for accurate coding in a remote team.

Qualifications

• 2+ years of facility-based outpatient coding experience.
• Experience coding Emergency Department encounters (E/M).
• Knowledge of injections and infusion coding in outpatient or hospital settings.
• Familiarity with federal/state/payer coding regulations.

Responsibilities

• Review documentation and assign ICD-10-CM diagnosis and procedure codes for outpatient settings.
• Use encoder applications to code per policies and guidelines.
• Query physicians to clarify diagnoses and procedures as needed.
• Perform peer reviews to support quality and consistency.
• Prepare coding-related reports and maintain high accuracy and productivity.
