Job summary

UnitedHealthcare is hiring a Field Care Coordinator - HIDE SNP in Cass County, MI. This field-based role supports LTSS recipients through person-centered care planning, coordination, and services across the continuum.

Travel up to 75% within a 50-mile radius, with telecommute flexibility for residents of Michigan. You will collaborate with care teams, providers, and families to ensure timely, cost-effective, and culturally relevant care that meets member needs.

Qualifications

• Must possess one of the following: Master’s degree and current, unrestricted independent licensure as a Social Worker (e.g., LMSW, LCSW, LLMSW)
• Bachelor’s degree and current, unrestricted independent licensure as a Social Worker (e.g. LLBSW, LBSW)
• 2+ years of experience in community health in a health care setting
• 1+ years of experience with local behavioral health providers and community support organizations addressing SDoH
• 1+ years of experience with long-term care needs and/or home and community-based services
• 1+ years of experience working with MS Office (Outlook, Excel, Word)
• Access to a quiet home workspace with PHI security
• Ability to travel to Southfield, MI for quarterly team meetings
• Must reside within Michigan
• Access to reliable transportation and valid US driver’s license allowing 75% travel within territory

Responsibilities

• Develop and implement care plan interventions as a single point of contact
• Communicate required health information to stakeholders for coordinated care
• Advocate for patients and families with the health care team
• Assess and implement individualized care strategies
• Identify problems/barriers to care and provide care management
• Refer to social service programs (financial, housing, etc.)
• Provide resource support for local services
• Manage the person-centered service plan across care continuum
• Conduct home visits with care team
• Conduct in-person visits (nursing homes, hospitals, home)
• Document member information to ensure compliance
