Job summary

Qualifications

• Current, unrestricted licensure as RN in Michigan or social work licensure as LMSW/LCSW/LLMSW or LBSW/LLBSW.
• 2+ years in community health setting or related health care role.
• 1+ years with local behavioral health providers addressing social determinants of health (SDoH).
• 1+ year with long-term care needs or home- and community-based services.
• Experience with electronic documentation systems and MS Office.

Responsibilities

• Develop and implement care plan interventions as a single point of contact across care continuum.
• Communicate health-related information to stakeholders to ensure quality, coordinated care.
• Advocate for patients and families to align with health care team decisions.
• Assess and implement individualized care strategies targeting appropriate levels of care.
• Identify barriers to care and initiate appropriate care management interventions.
• Refer to social service programs (financial, psychosocial, housing, etc.).
• Support members with local resources and PHI-protected information.
• Manage the person-centered service plan throughout care continuum.
• Conduct home visits and in-person visits to various settings.
