UnitedHealthcare, a part of UnitedHealth Group, is seeking a Field Care Coordinator- HIDE SNP in Michigan. The role centers on person-centered, goal-oriented care planning for members receiving LTSS, including case management and discharge planning, with a focus on social determinants of health.

The position is field-based with a home office and up to 75% travel within a 50-mile radius. Telecommuting may be possible in Cass County and surrounding areas.

Qualifications

• Must possess a Master’s degree and current, unrestricted licensure as a Social Worker (LMSW/LCSW/LLMSW).
• Must possess a Bachelor’s degree and current, unrestricted licensure as a Social Worker (LLMSW/LBSW).
• 2+ years of experience working within the community health setting in a healthcare role.
• 1+ years of experience with local behavioral health providers and community support organizations addressing SDoH.
• 1+ years of experience working with persons with long-term care needs and/or home and community-based services.

Responsibilities

• Develop and implement care plan interventions throughout the continuum of care as a single point of contact.
• Communicate with stakeholders the required health information to ensure quality coordinated care.
• Advocate for persons and families to ensure patient needs are represented by the health care team.
• Assess, plan, and implement individualized care strategies.
• Identify problems/barriers to care and provide appropriate care management interventions.
• Identify and initiate referrals for social service programs and resources.
• Provide resource support for local services based on assessments.
• Manage the person-centered service plan throughout care continuum.
• Conduct home visits in coordination with the person and care team.
• Conduct in-person visits to facilities or home settings.
