Job summary

Optum in Michigan is seeking a Field Care Coordinator to support LTSS through person-centered, goal-oriented care planning and coordination. This field-based role includes a home office and travel within a 50-mile radius, with some visits to facilities or hospitals.

Qualifications

• Current, unrestricted RN license in Michigan.
• Master's degree and current licensure as a Social Worker (e.g., LMSW, LCSW, LLMSW).
• Bachelor's degree and current licensure as a Social Worker (e.g., LLBSW, LBSW).
• 2+ years of community health experience in a health care role.
• 1+ years of experience with local behavioral health providers addressing SDoH.
• 1+ years of experience with long-term care needs or home and community-based services.
• 1+ year experience with electronic documentation systems and MS Office.

Responsibilities

• Develop and implement care plan interventions throughout the continuum of care as a single point of contact.
• Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously.
• Advocate for persons and families to ensure needs and choices are represented and supported by the health care team.
• Assess, plan, and implement individualized care strategies targeting appropriate levels of care.
• Identify problems/barriers to care and provide care management interventions.
• Identify and initiate referrals for social service programs including financial, psychosocial, housing, and transportation.
• Provide resource support to members for local resources based on service assessments.
• Manage the person-centered service plan throughout the continuum of care.
• Conduct home visits in coordination with the person and care team.
• Conduct in-person visits at nursing homes, assisted living, hospital, or home.
• Gathers, documents, and maintains all member information to ensure compliance with state/federal guidelines.

Education

RN license in Michigan

MSW / LMSW / LCSW / LLMSW licensure

BSW / LSBSW licensure
