Job summary

Foundation Communities seeks a compassionate Case Manager for Parker Lane in Austin, TX, supporting residents in the Children’s HOME Initiative. You will oversee 10–20 households, coordinate services, and liaise with property management and staff to ensure needs are met and housing stability is maintained.

You will collaborate with a multidisciplinary team, provide referrals, conduct goal setting, and track progress while honoring client self-determination and rights within a 24‑month supportive

Qualifications

• Bachelor’s Degree in social work, counseling, psychology, or related field.
• Minimum of two years’ experience providing trauma-responsive services to diverse or unhoused populations.
• Ability to work independently and as part of a multidisciplinary team.

Responsibilities

• Manage a caseload of families and provide case management and referrals.
• Meet regularly with Children’s HOME Initiative families to identify needs, set goals, and evaluate progress.
• Coordinate with other social service providers and educational opportunities.
• Ensure housing stability by overseeing compliance with program and property guidelines.
