Job summary

Foundation Communities seeks a Case Manager for Parker Lane in Austin, TX to oversee a caseload of residents in the Children’s HOME Initiative and coordinate services with property management and education staff. You will support families, conduct goal planning, facilitate access to resources, and ensure compliance with program guidelines while working with a multidisciplinary team.

This full-time role requires a Bachelor’s degree in a relevant field and at least two years of trauma-responsive

Qualifications

• Bachelor’s degree in social work, counseling, psychology, or related field.
• At least two years of experience providing trauma-responsive services to diverse and/or unhoused populations.
• Proven ability to work independently and as part of a multidisciplinary team.

Responsibilities

• Manage a caseload and provide case management and referrals to residents in the Children’s HOME Initiative.
• Meet families to identify needs, set goals, and monitor progress toward stability using evidence-based practices.
• Advocate for client needs, provide crisis intervention and de-escalation as needed.
• Coordinate educational opportunities and access to resources with partner providers and FC programs.
• Maintain documentation and participate in required meetings and trainings.
