Heartland Family Service is seeking a Community Based Crisis Navigator to join the Iowa Mental Health Crisis Response Team in Council Bluffs, IA. This full-time, on-site role delivers crisis intervention, risk assessment, and referrals to appropriate care levels, with flexible weekday/evening/weekend coverage.

Qualifications

• Bachelor’s degree in psychology, social work, nursing, education and 1 year of behavioral/mental health experience.
• Or high school diploma with 2 years in the human services field.

Responsibilities

• Provides crisis intervention and risk assessment for referrals.
• Makes referrals to appropriate levels of care after clinical consultation.
• Conducts crisis-focused assessments within 24 hours of crisis call.
• Establishes intervention plans with client and referral sources.
• Maintains clinical records meeting timeliness and completeness.
• On-call during daytime hours and responds on-site as needed.
• Develops connections with community resources and partners.
